All requests for additional TA resources, regardless of the source of additional TA support (for example, Dean's Reserves, New Enrollment Monies, Office of Undergraduate Education, Office of the Provost, Graduate School, etc.) must be submitted on this form to the Dean of Arts & Sciences Main Office.
